This grants program funds work that supports the Common Ground Healthcare Foundation’s mission to enhance health and prosperity in the communities where Common Ground Healthcare Cooperative’s members live, work, learn, and play.

ELIGIBILITY REQUIREMENTS

All applicants must meet the following:

  • Be a nonprofit organization with a 501(c)(3) designation by the Internal Revenue Service or a governmental body, such as a school or municipality, or an organization that is not tax exempt under I.R.C. § 501(c)(3). Grants to entities that are not tax-exempt under I.R.C. § 501(c)(3) shall be made only if the grantee organization agrees in writing to keep the funds in a separate fund dedicated to purposes described in I.R.C. § 170(c)(2)(B) 
  • Directly or indirectly serve the individuals within the Common Ground Healthcare Cooperative service area, Brown, Calumet, Dodge, Door, Fond du Lac, Green Lake, Jefferson, Kenosha, Kewaunee, Manitowoc, Marinette, Milwaukee, Oconto, Outagamie, Ozaukee, Racine, Shawano, Sheboygan, Walworth, Washington, Waukesha, Waupaca, Waushara, or Winnebago counties
  • Conduct business without discrimination on the basis of race, religion, color, sex, gender identity, sexual orientation, age, marital status, disability, veteran status or national origin

TYPES OF GRANTS

  • One-year or multi-year grants
  • Program or project support
  • Unrestricted grants for organizations closely aligned with Common Ground Healthcare Foundation’s mission

GRANTMAKING CRITERIA

Projects will be evaluated on the following criteria. Strong programs will:

  • Enhance health or prosperity of individuals in at least one of the 24 counties served by Common Ground Healthcare Cooperative
  • Articulate a well-defined need with clearly defined actions to meet the need
  • Build organizational capacity and lead the program to become self-sufficient
  • Use collaboration to leverage impact
  • Be feasible and likely to succeed
  • Have a reasonable level of risk
  • Propose an accurate, appropriate and reasonable budget
  • Attract additional funding and leverage resources
  • Clearly define meaningful and measurable project outcomes and methods to report actual outcomes
  • Have a long-term impact, especially relative to cost

WHAT WE DO NOT FUND

The Common Ground Healthcare Foundation does not fund the following:

  • Annual appeals or other fundraising activities or events
  • Religious activities or programs that serve, or appear to serve, specific religious organizations, groups, denominations, or beliefs
  • Individuals
  • Endowments
  • Lobbying or legislative activities
  • Programming or expenses occurring before the grant is awarded
  • Efforts substantially serving people outside of Brown, Calumet, Dodge, Door, Fond du Lac, Green Lake, Jefferson, Kenosha, Kewaunee, Manitowoc, Marinette, Milwaukee, Oconto, Outagamie, Ozaukee, Racine, Shawano, Sheboygan, Walworth, Washington, Waukesha, Waupaca, Waushara, or Winnebago counties.
  • Organizations who seek funding to disburse to a secondary-nonprofit organization

GRANT AWARDS

Typical grants are between $5,000 and $50,000.

Innovative work that closely aligns with Common Ground Healthcare Foundation’s mission may be funded over multiple years.

APPLICATION TIMELINE

Application Opens: September 1
Application Closes: November 1

Decisions will be announced within 8-10 weeks of the application deadline. All applicants are strongly encouraged to contact Kari Olsen, Director of Grants and Scholarship Programming, with any questions.

FINAL REPORT

Grant recipients will be required to complete a brief follow-up report at the end of the project or grant period.
